AUTHORIZATION OF CUSTODY OF MINORS

Hereby Swear and Affirm

2.         XYZ born on 00.00.0000 here at  city and country.   

Whereas due to some domestic disputes arisen in our marital relationship unfortunately we both could not live together which resultantly subsequently this knot come to an end in shape of divorce on 00.00.0000. Since  (name)  lives in United States of America, he wants to take his daughters with him for the welfare of them and for what I have no objection herewith. I hereby give my full consent to hand over the custody of my daughters to my Ex-Husband who is the father and de facto guardian of my daughters and having a lot of love and affection with them.
